Either the doctor or the nurse could authenticate it. They are the ones with personal knowledge of the event (the creation of a medical record).
For either witness, the questions are:

1) Are you familiar with this report? (personal knowledge)

2) Is it a report on Fred Turner’s treatment at the Emergency Room on September 25, 2015? (authentication)
